Consortium Overview

Our consortium exists to provide an array of caregiving services and support to the caregivers in our communities. We strive to create a strong network of caregiving agencies that will be qualified and prepared to meet the growing need for home based support.

The mission of the Consortium is to serve as a regional network of organizations committed to providing support services for caregivers and/or caregiving programs; to foster cooperative efforts in developing volunteer recruitment, public awareness, media relations, public relations and fund raising campaigns in our regions.

Catholic Charities Caregiver Support Services
Catholic Charities Caregivers Support Services is an agency of Catholic Charities of the Diocese of Albany with one simple mission - to make life better for those who care for friends and relatives in need.  Catholic Charities Caregiver Support Services provides two programs, the Elder Caregiving Program and the Kinship Care program.  The Elder Caregiving Program supports caregivers as they care for their older spouses, parents and friends.  This program provides respite service, information and assistance, social work services and support groups.  The Elder Caregiving Program provides services to the counties of Albany, Columbia, Delaware, Fulton, Greene, Herkimer, Montgomery, Otsego, Rensselaer, Saratoga, Schenectady, Schoharie, Warren and Washington Counties.  The Kinship Care program  focuses on supporting relatives who are raising children.  This program provides respite service, information and assistance, social work services, support groups and social and recreational activities.  This program services Albany and Rensselaer Counties.

Community Caregivers, Inc-Bethlehem and Guilderland
Community Caregivers consists of over 450 neighbors who can spare a little bit of time for others in our neighborhood who need a helping hand.  Community Caregivers helps: Older residents, People with chronic illness, Single and teen parents, and Caregivers who need respite. Community Caregivers serves people of all ages by providing the following free services: transportation, shopping, caregiver respite care, meal preparation /delivery, light housekeeping, chores, assistance with paying bills and correspondence, referrals to other agencies and services, visiting, library book delivery and technology assistance.

Eddy Alzheimer's Services Care Team Program at Marjorie Doyle Rockwell Center
Marjorie Doyle Rockwell Center is proud to sponsor the Alzheimer's Care Team (ACT) Program, instituted in 2002 through a grant from the Robert Wood Johnson Foundation. The program provides valuable non-medical assistance, support, and friendship to individuals with memory disorders and their family members and is part of a national Faith in Action Program.
An Alzheimer's Care Team consists of five to nine caring individuals, formed from within a faith-based community, i.e. a church, temple, synagogue or other congregation.  As part of a care team, members provide only those services that they're comfortable providing, and when it's convenient for them. The team approach offers volunteers the support of other fellow team members. And, since the whole team shares the tasks, no one has to do it alone!

Eddy Visiting Nurse Association- AIDS and Elder Care Teams
The Eddy VNA Care Team Program provides non-medical support and socialization to an Eddy VNA patient in a five county area encompassing not only the Capital Region but the Catskills as well. This program is unique because a team of 4 to 7 volunteers provide support to one patient who is referred to as a care partner. This team approach makes it easy for those with busy schedules to volunteer because you pick what you want to do and at a time that fits your schedule. Supportive services include transportation to appointments, running errands, preparing a meal or special treat, phone calls, home visits, and socialization. Socialization has become an important part of the program as care partners live alone at home with little or no family support.  Care Teams take their care partners out to lunch, on picnics, to recreational and cultural events, and other community activities. They are right there for their care partners as well during the holidays and on birthdays celebrating in *true care team * fashion. 25% of the volunteers have been with the program 10 years or more while 50% of our volunteers have been with the program 5 years or more. As a Faith In Action program, we have historically recruited from faith-based communities, but we also recruit from businesses, community groups, and colleges.

Senior Services of Albany
Senior Services of Albany, throughout its 55 years, has a proven history of innovation in responding to the needs of the elderly. As pioneers in the field of aging since 1952, Senior Citizens Centers, as it was known then, began as one of the first agencies to serve the elderly in the Capital Region. With donated space from the YMCA, temporary cash support from the Council of Jewish Women and the Junior League of Albany and an in-kind program director provided by the city adult education funds, the first senior center began providing social, recreational, educational and health activities. The center accepted membership and support from the Community Chest in 1954 and continued to grow and add programming through the '50s and '60s.
Senior Services of Albany provides: Transportation and Nutrition Services, Caregiver Support, ElderSource Line (our Information Hotline), Adult Day Care, Trips, Classes, Health Promotion and Wellness Programs, and Community Events.

Columbia County Community Healthcare Consortium, Inc.
The Healthcare Consortium is a rural health network made up of your friends and neighbors from the health care, business and consumer sectors. It is one of 34 such networks throughout New York State and receives funding from the New York State Department of Health, Office of Rural Health, and through public and private resources and donations.
The Healthcare Consortium has been improving access to health care for Columbia County residents with programs and initiatives that meet identified community needs.  The Medical Transportation System which provides Transportation service to and from health appointments for residents with no other means to get there.

Catholic Charities, established in Herkimer County, in 1980, is an agency of Catholic Charities of the Diocese of Albany.  The mission of Catholic Charities is threefold: to maintain the dignity and improve the quality of life of families and individuals in Herkimer County; to provide services top those in need without distinction; and to impact systems to become more aware and responsive to human needs.  Catholic Charities sponsors multiple programs, including the Retired and Senior Volunteer Program and the Faith in Action Program of Catholic Charities.

Care Links Community Caregivers
Shenendehowa Senior Citizens, Inc.
Care Links is a creative Community Caregivers Program providing non medical care to neighbors 55 and over who are in need and respite relief for those responsible for their care.  Trained volunteers provide, at no cost, supportive services such as: Friendly Visiting, Information and Referral, Light Housekeeping, Grocery and Other Shopping Assistance, Meal Preparation, Medical and Non Medical Transportation, Respite Assistance, and Telephone Assurance.  The Care Links Community Caregivers Program is geographically defined by the boundaries of the Towns of Clifton Park, Halfmoon, and Malta.

Mechanicville Area Community Services Center, Inc.
Based in the Mechanicville Senior Center, the Elder Care Services Program (ECSP) assures basic social and physical needs of area seniors are met. In its first year of operation alone, the Elder Care Services Program has established several well received programs.  Elder Care Services Program provides assistance in educational, enrichment, health and social programs to all individuals sixty years old and above. This program includes the following: information and referral support for senior caregivers, one to one counseling with the Elder Care Services Coordination or working in conjunction with other area agencies and family members to establish stronger support networks.

RSVP's Neighbor-to-Neighbor Program
RSVP's Neighbor-to-Neighbor Program matches caring volunteers aged 55 and over to community members who may need help to remain independent and living in their own homes.  Trained volunteers provide voluntary services such as information and referral, friendly visiting, friendly phone calling, light housekeeping or meal preparation, grocery shopping, and transportation to medical or medically related appointments. All Neighbor-to-Neighbor Program services are directed to individuals residing in Ulster County.

Catholic Charities Caregivers Support Services is an agency of Catholic Charities of the Diocese of Albany with one simple mission - to make life better for those who care for friends and relatives in need.  Catholic Charities Caregiver Support Services provides two programs, the Elder Caregiving Program and the Kinship Care program.  The Elder Caregiving Program supports caregivers as they care for their older spouses, parents and friends.  This program provides respite service, information and assistance, social work services and support groups.  The Elder Caregiving Program provides services to the counties of Albany, Columbia, Delaware, Fulton, Greene, Herkimer, Montgomery, Otsego, Rensselaer, Saratoga, Schenectady, Schoharie, Warren and Washington Counties.
